Zapier allows you to automatically send new Formstack Submissions into AMPED as leads.  Use the template to get started!

Getting Started:

To use this template, you will need the following:

  • Zapier Account
  • Formstack Account
  • AMPED Account

How to Setup the Template:

Click the “Use this Zap” button next to the template Name “Create AMPED leads from new Formstack submissions” to begin configuration of the template.

The Formstack Trigger “New Submission” will be automatically selected.  Click “Continue” to move onto the next step.

Once the trigger has been selected, you must connect your Formstack account.  Clicking “Connect an Account” will populated anew window where you will enter your Formstack account information.  If the window does not appear, check your browser pop-up settings.

Zapier requires two API Permissions to use Formstack in a zap; Allow read/write access to all your forms and read access to all of your form submissions.  Enter you Formstack account login credentials to authenticate your account and allow Zapier the API Permission.

Once your Formstack credentials have been entered, click “Authorize” to grant Zapier access to your account.

Once your Formstack account has been connected to Zapier, you will see a green “Success!” notification.  Click “Save + Continue” to move forward.

Now you must select which form you want Zapier to watch for new submissions.  Click the dropdown arrow to display all available forms.  If you do not see the desired form, click “Refresh Fields” and try again. 

If you would like to perform a test to make sure the connection to Formstack is working, you must have at least one form submission.  If you do not have any submissions, click “Skip This Step”.

Zapier will provide a default sample you can use to test the connection if you do not wish to use your own samples (you own submissions).

 Now it is time to connect your AMPED account.  This template will have the AMPED Action “Create Lead” selected by default.  Click “Continue” to connect your account.

If you have already connected an AMPED account to Zapier, it will be displayed in the list.  If not, click “Connect an Account” to authenticate your AMPED account.

Enter your AMPED username and password to connect it to Zapier.

When your AMPED account has been connected, you will see a green “Success!” notification.  Click “Save + Continue” to move onto the next step.

You will now be prompted to map the fields from your Formstack Form to your AMPED fields.  Click on “Show advanced options” to display all available fields.

Every owner within your AMPED site will be available to select unless other permissions have been established.  Select the owner you want the new leads to be assigned to and then map the remaining fields below.

Once your fields have been mapped, you have the option to send a test lead into AMPED.  Clicking “Send Test To AMPED” will send a sample lead into your AMPED account to ensure the zap is performing as expected.  You may also “Skip Test” if you do not wish to have the sample lead sent.

After having sent the test lead to AMPED or skipping that step, you are ready to create the zap!  Click “Finish” to finalize this zap template.

Congratulations!  You have now configured your Formstack-AMPED zap!  Now you can automatically add new Leads in AMPED from Formstack Form Submissions.
